Please note: If you volunteered for the Tutoring Program in the 2024-2025 school year, you DO NOT need to complete the below steps. Staff will be in contact with you soon.

1. Download and complete the TWO Background Authorization Forms below. Please read and follow the steps for successful completion of these forms:
   • Download Criminal Background Check Form #1
     • This form may be completed and signed electronically.
   • Download DCFS Background Check Form #2
     • This form MUST be printed out and signed with a PEN. Digital or electronic signatures are NOT accepted.
     • If you return this form via email, it must be scanned back in and emailed as a PDF. Photos of the forms are NOT accepted.
   • Please return your completed forms via email to tutor@chicagolights.org or in person at 126 E. Chestnut St. Chicago, IL 60611 c/o Mark Dennison.
2. Send this Reference Form Link to two individuals in your life that know you well and can speak to your character: https://chicagolights.tfaforms.net/15 .
3. Download and complete The Child Protection Policy Training. Please return the last page to tutor@chicagolights.org.
